Sept. 27: Food inspections

Reports from the Madison County Health Department, based on recent inspections of restaurants and other places that serve food. A critical violation is one that could significantly contribute to contamination, illness or environmental hazard. A non-critical violation is a minor violation, unlikely to contribute to contamination, illness or environmental hazard. Potentially hazardous food is food that needs to be kept below 31 degrees or above 135 degrees. Restaurants may have corrected violations between the time of the inspection and the publication of this report. All reports are from routine biannual inspections, unless otherwise indicated.
(*) Indicates violation repeated from earlier inspection.
(***) Denotes items found on last four inspection. Any food item not date-marked during next inspection will be destroyed by inspector.
JOHN’S DINER, 117 W. Washington St., Alexandria - July 30
Critical violation: Employee handling ready-to-eat foods with bare hands (making hamburgers).
Non-critical violations: (1) Walls beside deep fryer and floor under grill very soiled. (2) Outside of grill and shelf beside it very soiled.
DAIRY QUEEN, 501 Scatterfield Road, Anderson - Aug. 3
Critical violation: Bucket of coleslaw found in walk-in cooler, next to handsink, not date marked.
Non-critical violations: (1) Cheese found not covered, in walk-in cooler next to handsink. (2) Floors under and around equipment soiled.
LEMON DROP, 1701 Mounds Road, Anderson - Aug. 3
Non-critical violations: (1) Owner used sink in restroom to wash hands, instead of designated handsink. (2) Restroom door found propped open. (3) Container of lettuce found in cooler, next to grill, covered with linen.
HARVEST MARKET #4, 19 W. Cross St., Anderson - Aug. 4

Critical violations: (1) (***) Lunch meat found not properly date marked. (2) Lunch meat being held past seven-day holding time. (3) Fruit found not properly date marked. (4) (***) Baby formula outdated: Enfamil (13 oz.) Aug. 1; Lipil (1 qt.) Enfamil Lactofree Lipil (13 oz.) July 1; Similac Isoil (13 oz.). (5) Handsink in meat cutting area inaccessible and not in good working order. (6) Chemical bottle containing chemicals found hanging on single service item rack in meat cutting area. (7) Slicer, located next the meat display, not being properly maintained during shift. (8) Raw meat found being displayed above ready-to-eat foods in meat display case.
Non-critical violations: (1) (*) Thermometer not present in fruit and produce displays. (2) Thermometer in dairy display broken. (3) Single service items found stored on floor, next to sink, by the back door. (4) Boxes of food were being stored directly on floor in walk-in freezer.
PAY LESS SUPER MARKET, 1825 N. Scatterfield Road, Anderson - Aug. 5
Critical violations: (1) Sliced chicken and turkey found at 61º in deli department. (2) Bottle of Windex stored next to single service items in bakery.
Non-critical violations: (1) (*) Floor of dairy cooler, under the milk display racks, soiled. (2) Thermometer in dairy freezer not in good working order. (3) Thermometer in fish display not in good working order.
UP YOUR ALLEY LOUNGE, 2521 E. 10th St., Anderson - Aug. 5
Critical violation: Gallon of milk in bar area out-dated.
Non-critical violation: Ice scoop not being properly stored on top of ice maker.
McDONALD’S OF ALEXANDRIA, 1503 N. Park Ave., Alexandria - Aug. 6
Critical violation: Large fly population throughout kitchen area. Employee explained that back door was left open for an extended time during bread delivery.
Non-critical violations: (1) Single service items stored directly on floor. (2) Thermometer in refrigeration not in good working order. (3) Wiping cloths not being kept in sanitizing solution between uses.

Critical violations: (1) (***) Potentially hazardous ready-to-eat food in refrigeration not properly date-marked. (2) (***) Handsink blocked by soiled linen hamper.

Non-critical violations: (1) Ice scoop lying directly in ice, in coke machine at drive-through window. (2) Ice scoop found stored directly on ice maker. (3) Cabinets under pop machine in dining room extremely soiled.
BURGER KING 8468, 2910 Broadway, Anderson - Aug. 11
Critical violation: Backflow prevention device not present on mop sink by back door.
Non-critical violation: Back door found propped open.
CAPTAIN D’s SEAFOOD, 2701 Broadway, Anderson - Aug. 11
Critical violations: (1) Black substance found inside ice maker. (2) Employee drink found with no lid or straw, next to soup warmers. (3) Potentially hazardous foods found in walk-in cooler not date marked (noodles, cheese, baked potatoes, etc.). (4) Raw fish found stored above ready-to-eat foods. (5) (*) Cooler under small make table running above 45º; baked potatoes 47º. (6) Detergent being used to clean tables in dining area.
Non-critical violations: (1) Fish not covered in walk-in cooler. (2) Corn on cob and slaw in walk-in cooler and boxes of food in walk-in freezer found stored on floor. (3) Styrofoam cup without a handle stored in chili sauce. (4) Single service items stored on floor in outside storage closet. (5) (***) Back door does not properly seal at bottom. (6) Door of outside storage room does not seal properly. (7) Large grease build-up under deep fryers. (8) Large amount of dust and grease build-up on ceiling lights above food prep area. (8) No thermometer under make table, on food prep line. (9) Small freezer door falling off refrigeration unit. (10) Floors in food prep area no longer smooth surface, due to missing grout.

Critical violation: Bucket of dough had spilled over and dough was lying directly on floor.
Non-critical violations: (1) No soap with handsink, next to deep fryer. (2) Floor in storage room soiled, with food debris on it. (3) No thermometer in freezer. (4) Bathroom door standing open, with no self-closing device.
TACO BELL, 2618 Broadway, Anderson - Aug.. 11
Non-critical violations: (1) Back door standing open (with flies present). (2) Sliding door on Dumpster found open.
