Reports from the Madison County Health Department, based on recent inspections of restaurants and other places that serve food. A critical violation is one that could significantly contribute to contamination, illness or environmental hazard. A non-critical violation is a minor violation, unlikely to contribute to contamination, illness or environmental hazard. Potentially hazardous food is food that needs to be kept below 31 degrees or above 135 degrees. Restaurants may have corrected violations between the time of the inspection and the publication of this report. All reports are from routine bi-annual inspections, unless otherwise indicated.

All food and self-serve items must be stored at least six inches above floor.
El Chile Poblano, One Plaza Dr., Pendleton - March 10
Critical violations: 1. (*) Hand sink in server station blocked with a bucket of sanitizer. 2. Sanitizer water had no sanitizer solution in it. 3. (*) Hand sink in kitchen found with towels and sponges in it. 4. Hamburger and melted cheese on make table were at 118º and 112º, respectively. 5. (*) Certified food handler not present. 6. Shrimp found in direct contact with ice, as cooling method.
Non-critical violations: 1. Powder substances (white, brown and tan) found not labeled. 2. Employee observed handling ready-to-eat foods without gloves. 3. Black substance found inside ice maker.

Non-critical violations: 1. Racks in dry storage area found very dusty and sticky. 2. Mop sink not draining.
Mancino’s, 1606 Scatterfield Rd., Anderson - April 17
Critical violations: 1. Bottle containing chemical found with no label. 2. Sliced ham and beef found in walk-in cooler being held past 7 day period. 3. Employees found eating a pizza that was on the wooden make table.
Non-critical violations: 1. Ceiling tiles above make table and in chemical room were missing. 2. No thermometer found under vegetables on make table. 3. No thermometer provided in freezer unit next to 3-bay sink. 4. Black substance found growing on inside of ice maker.
Pendleton Elementary School, 327 S. East St., Pendleton - April 17
Critical violation: 1. Hash browns found on serving line at 117º.
Valley Grove Scool, 2100 S. 300 East, Anderson - April 27
No violations found.
Bejing City Buffet, 3035 W. U.S. 36, Pendleton - April 30
Critical violations: 1. Bottles of soap found next to raw shrimp on sink. 2. (*) Eggs on prep. table found at 72º. 3. (*) Pan of chicken found under prep table. 4. (*) Can opener found very soiled. 5. Raw chicken in walk-in cooler found uncovered. 6. Chicken wings found on buffet at 99º. 7. (R) No sanitizer water was present in kitchen. 8. Several items found uncovered in reach-in cooler. 9. Possible cross contamination found - shrimp being stored with raw chicken on prep. table. 10. Pan of whole green peppers found sitting directly on top of pan of cut green peppers and onions.
Non-critical violations: 1. (*) Soap and paper towels not provided at hand sink in kitchen. 2. (R) Tub of chicken found stored directly on floor of walk-in cooler. 3. Several tubs of packaged beef found stored on floor in freezer. 4. Many food items found uncovered in refrigeration unit. 5. Wiping cloths not being stored in sanitizer between uses. 6. Reach-in refrigeration unit found leaking. 7. Plate and bowl found sitting directly on top of raw pork. 8. One pair of tongs being used between raw pork, beef, chicken, etc. 9. No thermometer provided at prep. table. 10. There is an approximate 1 1/2” gap at top of back door.

Non-critical violation: 1. Thermometer in glass front refrigeration in kitchen found broken.

Non-critical violation: 1. Back door in the bar does not seal, and sliding door was open.

Critical violation: 1. Employees washing hands in 3-bay sink in back storage room: (Mop sink provided with soap and paper towels). 2. Hand sink in front is being used for purposes other than hand washing.
Non-critical violation: 1. Rest room door no longer self-closing and was standing open.

Non-critical violations: 1. Walk-in freezer in main kitchen at 20º. 2. Back door of Haven does not properly seal at the bottom.
